Preventable Readmissions

According to HealthCare.gov, a federal government website managed by the U.S. Department of Health & Human Services:

Hospitals, post-acute facilities, home care agencies, clinicians, and community-based organizations, regardless of where they provide care and services, may seek evidence-based programs and tools to help them improve the safety and effectiveness of care transitions, particularly for high risk individuals. Examples of well-established programs and tools are listed here.

Some of the interventions and tools address the process of the transition of care and others address common causes of problems following hospitalization (such as medication management or disease-specific management). The QIOSC and the Administration on Aging websites have additional resources and tools. Many of these resources and tools are useful for providers in all settings, while others are meant for a particular provider or setting (such as hospital, CBO, home care or post-acute facility).
