The dates for the second series are Nov. 11, Dec. 2, Dec. 14 and Jan. 11, 2017.

This webinar series will focus on:

• Managing readmissions in post-acute care
• The latest on episode payment models, including combined joint replacement (CJR) and the new mandatory cardiac bundling program proposed by the Centers for Medicare and Medicaid Services
• The effect of the IMPACT Act on acute and post-acute provider network development and provider collaborations
• The evolution of Medicaid managed care to value-based payment.

Content is appropriate for acute and post-acute care providers who are engaged in partnerships, network development, bundling, ACOs and other alternative payment models.
