May 19, 2016: Department of Health Issues Funding for Diabetes Initiatives


The N.J. Department of Health’s Heart Disease and Stroke Prevention Program has announced the availability of funding to support two initiatives.

Money will be made available in the 2017 budget cycle for:

  • Accountable care organizations to adopt strategies that promote awareness of high blood pressure among patients and promote awareness of prediabetes among people at high risk for Type-2 diabetes. Approximately $24,000 is available for each award.
  • Management of patients with high blood pressure and diabetes and support for a Clinical Decision Support System. Horizontally integrated healthcare organizations that have two or more acute care, specialty care or psychiatric care facilities are encouraged to apply for this competitive funding opportunity. Approximately $160,000 is available for the funding of one application.

The project period is nine months (Oct. 1 - June 30, 2017) with a nine-month budget period and an anticipated award date of Oct. 1. A Letter of Intent is due by noon, June 3, to LorieAnn Wilkerson-Leconte at LorieAnn.Wilkerson-Leconte@doh.nj.gov
