Nov. 24, 2015: Register Today: Dec. 4 Session Explores Health Info Exchanges in Post-Acute Care


There’s still time to register for NJHA’s Dec. 4 education program, "Long Term Care & Post-Acute Providers – Connecting to Healthcare Information Exchanges."

This day-long program will focus on enhancing clinical data exchange between hospitals, physicians, long term care, home health agencies, inpatient rehabilitation hospitals and long term care hospitals.

As the provider community transitions to value-based payments, the ability to share clinical data is critical to achieve improved outcomes, high quality patient experiences and better cost-effectiveness. Representatives from the six health information exchanges operating in the state will present on ways long term care and post-acute providers can connect or access timely patient clinical information.

An afternoon panel discussion of long term care and other post-acute providers will showcase facilities that have leveraged information from the health information exchanges to better serve their patients and residents.
