Nov. 20, 2015: Dec. 4 HIE Ed Session to Focus on Cross-Setting Data Sharing


HRET will host a full-day education program, "Long Term Care & Post-Acute Providers – Connecting to Healthcare Information Exchanges," Dec. 4 focusing on enhancing clinical data exchange between hospitals, physicians, long term care, home health agencies, inpatient rehabilitation hospitals and long term care hospitals.

All six New Jersey HIEs will be presenting on ways long term care and post-acute providers can connect or access timely patient clinical information. The long term care and post-acute provider industry segments were not included in Medicare and Medicaid Electronic Health Record incentive funding programs. To help bridge this critical informational gap, New Jersey HIEs offer alternative ways to access patient information for those facilities that are not able to share or download to an EHR of their own.  

As the provider community transitions to the value-based payment paradigm, the ability to share and use clinical data is critical to achieve improved outcomes, high quality patient experiences and better cost-effectiveness. CMS just finalized a mandatory bundling payment program for lower extremity joint replacements that will affect a significant number of hospitals and their post-acute partners. Now is the time to identify the best way to share data that can support safe and appropriate care redesign and transitions to succeed under this, and other, emerging payment models.

There will be an afternoon panel discussion of long term care and post-acute providers that currently subscribe to an HIE to share how they are able to leverage information for the patients they serve.
