Mar. 30, 2018: April 4 Program Targets Reductions in Medicaid Readmissions


NJHA’s Health Research and Educational Trust is hosting a day-long work session April 4 as part of its Reducing Medicaid Readmissions initiative. 

As the federally designated New Jersey Hospital Improvement Innovation Network (NJHIIN), HRET is leading a learning action collaborative aimed at reducing Medicaid readmissions. Reducing all-payer readmissions has been a longstanding focus of the NJHIIN, and now with new Medicaid readmission non-payment policies, organizations need to be prepared to implement practices aimed at preventing readmissions in this patient population.

To improve processes aimed at reducing readmissions, Dr. Amy Boutwell, author of the ASPIRE readmission reduction model, will lead New Jersey organizations through the steps of this framework using discussion, coaching and workshop-style learning. Dr. Boutwell will assist and guide participating hospitals in developing Medicaid readmission reduction goals and strategies, as well as implementation action plans.

This program is intended for readmission reduction teams – physicians, nurses, pharmacists, social workers, case managers, quality directors and at least one “Medicaid-relevant” non-hospital partner, such as behavioral health, peer support, community health worker or community-based social worker.
