Access to Primary Care in N.J.
This study investigates geographic variation of avoidable hospitalizations for certain conditions, identifies areas with highest hospitalization rates and determines access-related systemic issues and local barriers in each area. The primary goal of this study is to provide information needed to improve access of the medically indigent and uninsured populations to primary health care services throughout that state of New Jersey.

Breast Cancer Awareness

This project provides information about screening services and facilities throughout New Jersey through a comprehensive resource guide, highlighting those that provide services for low-income and high-risk populations free or at reduced fees, are available evenings and weekends, and offer culturally sensitive and handicapped accessible services. The goal is to improve women's access to early detection services through more referrals by primary care providers.

Building Healthier Communities: County Health Profiles

This publication, compiled about every five years, provides a profile of the community's health in each of 21 counties through county-specific snapshots of population and health statistics not routinely found in a single source. County-level data and rates are compared to state averages for benchmarking. The profiles serve as a valuable reference and resource tool for hospital planners, community agencies and offices of public health.

Community Benefit Tracking and Reporting
New Jersey hospitals provide compassionate, patient-centered care for all, regardless of their ability to pay. It takes more than medical care to make a community healthy; with each health service, preventive screening, educational program and dollar invested for community building and helping people in need, hospitals advance the health of their communities. These hospitals identify their community needs, partner with community organizations and take on leadership roles to implement programs that address issues well beyond the scope of their mission-driven patient care activities. NJHA demonstrates the value of New Jersey’s hospitals to the communities they serve through collecting data on their varied community programs and services and highlighting them in reports and tools for education and advocacy.

Community Outreach Awards
Annually, HRET recognizes hospitals with unique and effective methods of identifying and reaching out to better serve the healthcare needs of area residents. Lists award winners from 2003 to the present.

Comparative Clinical Outcomes

This report, released annually, provides actual and risk-adjusted predicted rates on quality indicators for three to five years of data, using hospital discharges. The goal is to support hospitals in their voluntary and mandatory outcomes measure reporting and assist them in their efforts to improve the quality of care rendered to their communities.

Covering Kids & Families

This project works to maximize enrollment of eligible, uninsured children in public healthcare coverage programs (Medicaid and NJ FamilyCare) through outreach, simplification and coordination activities conducted at both the statewide level and through four local community pilots. It is part of a national program, with the ultimate goal of reducing the number of children lacking health insurance coverage.

Dulce New Jersey: Diabetes Management Across Communities

Healthcare Interpreter Training Program for Bilingual Hospital Staff

Patient Race, Ethnicity and Primary Language Data Collection
This project studies hospital practices of collecting and coding data on patient race and ethnicity and develops educational interventions to address the needs. The goal is to support hospital efforts that would ultimately lead to accurate and reliable data reporting, assessment of needs/disparities in care and development of programs to address them.

Newborn Screening Education

This initiative develops and widely distributes educational resources on enhanced newborn blood and hearing screenings for parents and healthcare professionals. The goal is to prevent mental retardation and other serious medical or learning problems caused by certain inborn diseases through increased awareness and promotion of timely interventions.
