About Partnership For Patients

Partnership for Patients is a national initiative from the U.S. Department of Health and Human Services to improve the quality, safety and affordability of healthcare in the United States.

The two goals of this partnership are to make care safer by preventing hospital-acquired conditions and to prevent readmissions by decreasing complications during care transitions.


Creating a culture of safety is an ongoing journey. We share our successes and our struggles, learn from one another and come back every day with the passion to provide the safest care possible to our patients. Only through sharing this journey can we make the great – and important – strides in quality improvement our communities deserve.

The New Jersey Hospital Association has been selected as one of 16 organizations nationally to continue efforts in reducing preventable hospital-acquired conditions and readmissions through the Partnership for Patients.

The Hospital Improvement Innovation Networks are part of a broader effort to improve care for patients and reduce costs for Medicare.

Through 2019, NJHA and its members will work to achieve the national goals of a 20 percent decrease in overall patient harm and a 12 percent reduction in 30-day hospital readmissions from the 2014 baseline. Efforts to address health equity for Medicare beneficiaries and improving patient and family engagement in care will be central to the HIIN efforts.

This is the third time CMS has selected NJHA’s Health Research and Educational Trust for a Partnership for Patients initiative, a testament to the results achieved in New Jersey from 2012 to 2016:

  • more than 14,000 cases of patient harm averted
  • more than $120 million in healthcare cost savings
  • the second-best rate of reducing hospital readmissions in the nation, and
  • dramatic improvement in reducing infection and sepsis mortality rates, adverse drug events and other important quality measures.
