May 21, 2019: NJHA, VNA Offer Advance Care Planning Event


NJHA is partnering with VNA Health Group to offer a two-hour education program, Advance Care Planning: Discussing what matters most to you, June 19 at Harrogate in Lakewood.

The program will discuss the importance of advance care planning and communicating end-of-life wishes.  Residents, community members, legislators, legislative staff, social service workers, clinicians, attorneys, reporters and others are invited to have lunch at Harrogate Retirement Community to hear from: Dr. Marianne M. Holler, chief medical officer of hospice and palliative medicine for VNA Health Group; Dr. Sara Leonard, assistant medical director for Vising Physician Services; Kelley Ryan, social work manager for VNA Health Group; and Rabbi Zylbergerg from VNA Health Group.

Registration starts at 11:30 a.m. and the panel discussion will start promptly at 12 noon. The expert panel will discuss how completing advance care planning tools can improve care for individuals and their loved ones, and how palliative care, hospice and other advance care tools improve patient and family experiences.
