Two complimentary webinars for hospital care managers are scheduled for next month to review important trends and areas for improvement in Horizon Blue Cross Blue Shield of New Jersey’s Medicare Advantage plan’s post-acute care process.

Last year, Horizon Blue Cross Blue Shield of New Jersey’s Medicare Advantage plan teamed up with naviHealth so that all inpatient rehabilitation hospital and skilled nursing facility authorizations for patients discharged from hospitals would be handled through naviHealth. Now that a full year has transpired, naviHealth has identified ways to clarify and improve efficiency of the system.

The two complimentary webinars are set for June 8 at 10 a.m. and June 13 at noon; content will be the same on each date. The webinars will address:

• Clarification of the inpatient rehabilitation hospital and skilled nursing facility authorization processes
• Clinical documentation required to optimize efficiency
• Review of the acute care fax referral form to ensure proper communication
• Overview of related skilled nursing facility quality metrics assisting with readmission reduction, and
• Partners with which hospitals should work.
