P4P: Eliminating Preventable Readmissions and Improving Transitions in Care - 1236 (.pdf file size 75 KB 2 pgs.)

On August 18, 2011, CMS published final rules in the Federal Register which included a number of provisions to implement various sections of the Affordable Care Act. Among these provisions are the requirements for a hospital readmissions reduction program and related quality data reporting measures. Beginning in October 2012, inpatient prospective payments to a hospital will be reduced based on the dollar value of the hospital's percentage of preventable Medicare readmissions for three high volume procedures as chosen by the Secretary of HHS with the assistance of the National Quality Forum. This program is designed to bring effective strategies to hospitals to assist in eliminating preventable readmissions and improving transitions of care in high volume, high risk areas.

This activity provides continuing education credits for physicians and nurses.

This webinar will discuss how to move from collecting data and identifying gaps to action by setting priorities among identified community health needs. The Webinar will also showcase a national best practice model from St. Croix County in Wisconsin. Members of the Healthier Together – St. Croix County will present on a partnership between a hospital and public health department to conduct a community health needs assessment, the roles defined for each player in this assessment and the strategies used for identifying priorities and developing health improvement plans, as well as their successes and challenges.

I’m Still Here™: Montessori-Based Activities for Dementia - 1211 (.pdf file size 565 KB 2 pgs.)

This course focuses on I’m Still Here™: Hearthstone’s Montessori-Based Activity Program (MAP), an innovative method of working with persons living with cognitive impairments based on the educational philosophies of famed childhood educator Maria Montessori. This methodology has been researched for over 15 years by Dr. Cameron Camp and has been shown to significantly increase levels of engagement, participation, satisfaction, success and quality of life for persons living with dementia. Participants in this course will learn the fundamental principles of this methodology, in addition to the application of these principles using real-life examples drawn from their use at Hearthstone’s treatment residences.

Day 1 is required for all Day 2 participants

This activity provides continuing education credits for nurses, long term care administrators (LNHA/CALA), recreation therapists. An application for social work credit has been submitted to NASW-NJ.

This program will provide an overview of new models of hospital-physician alignment that are being utilized in New Jersey and throughout the country by hospitals and physicians in response to opportunities under the Affordable Care Act. These efforts such as ACOs and bundling, address the private sector’s demand for high quality, cost-efficient, coordinated healthcare services. Professional Services Agreements, affiliated or captive PCs, the “physician enterprise” model and co-management arrangements are being used with increasing frequency. There are numerous strategic, financial and legal issues raised by these various models that must be considered by providers and their legal and financial advisors.

Pressure ulcers are strongly associated with mortality and lengthy stays in acute, post-acute and long term care settings. Every year, pressure ulcers affect over 1 million patients and residents in hospitals and nursing homes. Costs associated with pressure ulcer treatment exceed $1.335 billion and of that, more than $355 million is spent on treatment in the longterm care setting. Hospital-acquired pressure ulcers are associated with a greater risk of death within one year of hospitalization. This conference will build on the great work that has been completed to date in New Jersey.
